Responsibilities

• Psychosocial evaluations
• Ongoing counseling of patients and families during their end-of-life journey
• Provide emotional support to patients and families when it's needed most
• Work in accordance with the plan of care

Requirements

• MS degree in social work from an accredited school of social work approved by the Council of Social Work Education
• Current state license as a social worker
• 2+ years of clinical work experience, preferably in healthcare or hospice
• Reliable transportation
• Ability to sit, stand, bend, move intermittently and lift at least 25lbs and bear the weight of an average adult effectively

• Strong interpersonal and communication skills
• Excellent problem-solving abilities
• Ability to work independently and as part of a team
