Responsibilities

  • Provide clinical intervention, including psychosocial, risk and level of care assessments, for patients identified as meeting high-risk criteria by Care Team members.
  • Complete the Case Management Initial Assessments for patients assigned to Social Work upon admission.
  • Provide consultation to members of the Care Team on bio-psychosocial, environmental and cultural factors.
  • Provide brief therapeutic intervention with patient and families around the issues of diagnosis, adjustment, grief or loss.
  • Utilize appropriate case management interventions to address current and/or future barriers to care and well-being; facilitate referral to support services to promote well-being and manage length of stay.
  • Upon patient admission, initiate the process of developing a discharge plan with patient, family and medical interdisciplinary team which will result in a safe disposition for the patient.
  • Facilitate family and treatment team meetings early in the admission, as clinically indicated.
  • Participate in supervision and team meetings.
  • Adhere to National Association of Social Work ethics and scope of service standards for Social Work Case Management practice.

Requirements

  • Four years of Social Work experience within a healthcare setting, psychiatric and substance use.
  • Knowledge of community resources.
  • Effective verbal and written communication skills.
  • Person-centered engagement skills.
  • Ability to build relationships with patients, family members, community collaborators and colleagues.
  • Brief treatment and crisis intervention skills.

Qualifications

  • Masters in Social Work (MSW).
  • Licensed Independent Clinical Social Work (LICSW).

Physical/Environmental Requirements

  • Frequent bending, carrying, pulling, pushing, reaching, standing, stooping, walking and lifting over 10 lbs.
  • Occasional need for climbing stairs, lifting in excess of 50 – 100 lbs and sitting.
  • Constant use of hearing, sight and speech.
  • Frequent use of smell and touch.
  • Frequent keyboard and telephone use.
  • Constant concentration and problem solving.
  • Moderate exposure risk to body fluids.

Pay

The pay range for this position is $28.90 - $51.34 per hour. This position is covered by a collective bargaining agreement. The pay range listed is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time and is established and governed by the terms of the applicable collective bargaining agreement.

As a health care organization, we require all staff to be vaccinated against influenza (flu) as a condition of employment.
