LICSW Social Worker

POSITION SUMMARY

Conducts comprehensive behavioral health assessments, diagnostic evaluations, and treatment planning for patients in the Emergency Department and inpatient settings, collaborating with interdisciplinary teams to support patient care and psychosocial needs.

POSITION DUTIES

  • Coordinates discharge planning and post-acute placement services, leveraging knowledge of skilled nursing facility (SNF) level-of-care criteria and regulatory requirements to ensure safe, appropriate, and compliant transitions of care.

Minimum Required Qualifications

  • WA LICSWM
  • Master’s in social work
  • 2 years of recent acute inpatient social work/case management experience required.
  • 4 years of overall social work experience required
  • Skilled experience in behavioral health assessment, diagnosis, treatment planning, discharge planning for post-acute placements (understanding SNF level of care, understanding regulatory requirements)
  • Travel experience
  • Epic experience
