Duties & Responsibilities

• Screen charts for historical and clinical information, conduct psychosocial evaluations through interviews with patients and/or identified surrogates, and formulate appropriate social work treatment plans.
• Provide individual and group counseling, as well as concrete social services, in health‑care setting offices or community settings throughout NYC.
• Consult with professionals in other disciplines to develop multidisciplinary treatment plans, evaluate patient progress, create discharge and after‑care plans, and recommend alternative actions or treatments.
• Deliver outreach services, make referrals to governmental, public, and community agencies, prepare patients to accept services, and provide direct social work services on high‑risk cases, including escorting patients for community activities and referrals as needed.
• Act as a mandated reporter by documenting and reporting suspected cases of child abuse or neglect to the New York State Central Registry.
• Appear and testify at administrative and court hearings when required.
• Assist in planning institutional or hospital discharge and other post‑institutional/hospital care; evaluate alternate care arrangements for patients as needed.
• Participate in staff conferences, social work meetings, special studies, committees, research projects, and internal/external quality‑assurance/performance‑improvement (QA/PI) activities, programs, and trainings.
• Prepare reports and related paperwork; maintain appropriate records.
• Ensure ongoing compliance with NYC Health + Hospitals policies, national standards, external regulatory requirements, accreditation authorities, and safety and quality management standards.
• May assist in the provision of clinical services under supervision of a New York State LCSW, licensed psychologist, or psychiatrist, or perform such functions independently as an LCSW.
• Mentor and assist less‑experienced social work staff in techniques; serve as a resource for advanced social work practice.
• Supervise graduate and undergraduate social work students, volunteers, and other auxiliary personnel as needed.
• Perform other related duties as directed, with required supervision.

Minimum Qualifications

• Valid license as an LMSW issued by the NYSED; or
• Valid license as an LCSW issued by the NYSED.

Preferred Knowledge, Skills, and Abilities

• Knowledge of substance use/abuse behaviors, interventions, and resources.
