Responsibilities

• Screen charts for historical and clinical information, conduct psychosocial evaluations through interviews with patients and/or identified surrogates, and formulate appropriate social work treatment plans.
• Provide individual and group counseling and concrete social services in health‑care settings or community settings within NYC.
• Consult with professionals in other disciplines to develop multidisciplinary treatment plans, evaluate patient progress, develop discharge and after‑care plans, and recommend alternative actions.
• Provide outreach services, make referrals to governmental, public, and community agencies, prepare patients to accept services, deliver direct social work services on high‑risk cases, and escort patients for community activities and referrals as needed.
• Act as a mandated reporter by documenting and reporting suspected child abuse or neglect to the New York State Central Registry; appear and testify at administrative and court hearings when required.
• Assist in planning institutional or hospital discharge and other post‑institutional care; evaluate alternate care arrangements for patients.
• Participate in staff conferences, social work meetings, special studies, committees, research projects, and internal/external quality‑assurance/performance‑improvement activities, programs, and trainings.
• Prepare reports and related paperwork; maintain appropriate records and ensure compliance with NYC Health + Hospitals policies, national standards, and external regulatory requirements.
• Provide clinical services under supervision of a New York State LCSW, licensed psychologist, or psychiatrist, or independently as an LCSW where authorized.
• Mentor and assist less‑experienced social work staff, serve as a resource for advanced social work practice, and supervise graduate/undergraduate social work students, volunteers, and auxiliary personnel.
• Provide direct clinical supervision and guidance to social work staff, screen and assign cases, schedule staff, review staff work, evaluate performance, and maintain time and leave records.
• Document clinical supervision logs and ensure timely completion of attestation forms required by the NYSED Office of the Professions for LCSW licensure applications.
• Conduct or lead special studies and projects, collect and evaluate data related to vulnerable patient populations, and prepare and present related reports.
• Establish and maintain professional social work standards for interview techniques, clinical assessments, treatment plan development, and case‑history documentation.
• Interpret clinical information, prepare comprehensive case reports, and submit findings to support recommended courses of action.
• Develop training curricula and present on social work concepts to members of other professional disciplines.
• Train and instruct staff in agency policies, procedures, and appropriate social work techniques, methods, and practices.
• Ensure compliance with accepted social work practices, government rules, regulations, and reporting requirements.
• Participate in conferences, committees, and meetings on‑ and off‑site with supervisory/managerial staff, clinical professionals, government representatives, and community organizations.
• Analyze population health issues, serve as liaison to community agencies, and facilitate communication and utilization of community resources.
• In the temporary absence of the supervisor, perform the duties of that position.

Qualifications

• Valid license as a Licensed Clinical Social Worker (LCSW) issued by the NYSED.
• Master of Social Work degree from an accredited graduate school of social work (preferred).
• Three (3) years of experience utilizing casework, group work, and/or community organization methodologies.
• One (1) year of supervisory experience, preferably in Behavioral Health services.
• Knowledge of psychopathology and various assessment and treatment modalities for mental health and substance‑use needs of diverse client populations.
• Familiarity with high‑risk adult and child patients of diverse backgrounds in community settings.
• Experience working with domestic‑violence and/or traumatized populations.
• Strong interpersonal and communication skills, both verbal and written, for interaction with patients, families, colleagues, multidisciplinary teams, managers, and administration.
• Ability to document interventions and care plans clearly, concisely, and legibly in the patient’s medical record.
• Ability to orient, supervise, and train employees and volunteers, including social workers and interns.
• Bilingual Spanish speaker (preferred).
