  • Manage and delegate Social Work referrals.
  • Deliver direct client support, including education, referrals, and brief case management.
  • Lead monthly Social Work team meetings.
  • Troubleshoot patient concerns and escalations in partnership with the Director.
  • Participate in departmental projects, goals, and continuous quality improvement initiatives.
  • Identify and implement workflow improvements.
  • Maintain timely, professional case documentation and ensure team-wide documentation standards are met.
  • Stay current with EMR workflows and changes affecting the Social Work department.
  • Represent and actively support the Social Work department throughout the organization.
  • Travel between Greater Seacoast Health clinical locations as needed.

Requirements

  • Bachelor’s degree in Social Work or a related human services field.
  • Excellent interpersonal and communication skills.
  • Ability to maintain confidentiality in all circumstances.
  • Strong computer literacy.
  • Ability to work independently and manage multiple priorities.
  • A client- and community-service mindset.
  • Ability to effectively lead groups and team meetings.
  • Strong problem-solving skills and the ability to respond to internal and external needs.

Preferred: Previous supervisory or team-lead experience, though not required. We seek candidates who can support, organize, communicate, and lead effectively.

Ideal Candidate

You may thrive in this role if you:

  • Enjoy being a trusted resource for teammates.
  • Can organize multiple moving pieces without losing sight of the people involved.
  • Communicate clearly and professionally, even in challenging situations.
  • Are comfortable making decisions and solving problems.
  • Balance team support with direct client services.
  • Adapt well to changing schedules, priorities, or patient needs.
  • Lead through collaboration, reliability, and respect.
  • Seek professional growth while continuing meaningful community-based work.
