Social Worker - LCSW - Lourdes Main Campus

Responsibilities

• Counsels and evaluates patients for psychosocial issues.
• Provides individual and group psychotherapy.
• Coordinates interdisciplinary assessment processes.
• Completes psychosocial assessments within organizational guidelines.
• Conducts group and individual therapy and/or case management activities.
• Manages family meetings as needed.
• Develops and implements patient treatment plans based on thorough assessments.
• Schedules work and patient activities with flexibility.
• Provides 24-hour on-call coverage and manages crises.
• Demonstrates professional judgment in reporting suspected abuse or neglect.
• Documents patient interactions, activities, and treatment plans.
• Explains and discusses legal documents like living wills and durable powers of attorney.
• Follows discharge planning, evaluation, and chart documentation policies.
• Develops instructional materials for cardiac rehabilitation patients.
• Collaborates with interdisciplinary teams to improve processes and services.
• Acts as an information and referral source for community agencies.
• Participates in quality improvement initiatives.
• Facilitates staff support groups and promotes a positive working environment.
• Participates in the development and implementation of the interdisciplinary treatment plan.
• Attends to treatment team activities and promotes a therapeutic milieu.
• Assists in the professional development of healthcare team members.
• Provides high-quality professional consultation and in-service education to medical and hospital staff.

Qualifications

• Licensed clinical social worker.
• Master's Degree.
• Crisis Intervention Training completed within 90 days of employment.
• Previous experience in social work.
• Proficiency in the English language, verbal and written communications, and basic computer skills.
