Responsibilities
- Counsels and provides individual and group psychotherapy.
- Develops and implements patient treatment plans.
- Participates in treatment team conferences.
- Serves as the case manager for assigned patients.
- Coordinates the interdisciplinary assessment process.
- Completes psychosocial assessments within guidelines.
- Conducts group and individual therapy and/or case management activities.
- Conducts family meetings as necessary.
- Develops and implements high quality patient treatment plans.
- Follows established practice guidelines for work and patient scheduling.
- Provides 24-hour on-call coverage for the center.
- Demonstrates professional judgment in reporting suspected abuse or neglect.
- Completes treatment plan documentation and updates.
- Documents all patient and caregiver interactions.
- Documents all group, individual, and family activities.
- Explains and discusses living wills and durable power of attorney.
- Fulfills discharge planning, evaluations, chart documentation, and follow-up coordination.
- Ensures appropriate and efficient completion of authorization to provide or receive information.
- Completes billing forms and writes letters and reports as needed.
- Develops high quality instructional materials for cardiac rehabilitation patients.
- Works collaboratively to solve problems, improve processes, and develop services.
- Acts as an information and referral source by developing cooperative agreements with community agencies.
- Participates in departmental and organizational quality improvement initiatives.
- Facilitates staff support groups and promotes a positive working environment.
- Participates in the development and implementation of the interdisciplinary treatment plan.
- Participates in treatment team activities.
- Promotes a therapeutic milieu by formal and informal methods.
- Acts as a clinical resource and serves as a mentor to health care team members.
- Identifies own learning needs and assumes responsibility for self-development in life-long learning.
- Provides high quality professional consultation and in-service education to medical and hospital staff.
- Performs other duties as assigned or requested.
Qualifications
- Must be licensed clinical social worker.
- Must have a Master's Degree.
- Must have completed Crisis Intervention Training within 90 days of employment.
- Must have previous experience in social work.
- Must have proficient verbal and written communication skills.
- Must have basic computer skills.
