Social Worker LCSW - Family Medicine
About the role
The social worker provides social work and behavioral health services for medical home clinic patients, collaborating with the care team, in accordance with policy and procedures of St. Patrick Hospital and Health Sciences Center and Providence Medical Group. The social worker performs assessments and documentation of presenting and historical behavioral health conditions, social and family history, psychiatric history, mental status, and other pertinent information. They also provide individually structured, targeted behavioral interventions for patients, working with adult and adolescent populations.
Responsibilities
- Performs assessments and documentation of presenting and historical behavioral health conditions, social and family history, psychiatric history, mental status, and other pertinent information.
- Provides individually structured, targeted behavioral interventions for patients, in coordination with the care team.
- Collaborates with the care team to ensure comprehensive patient care.
- Documents all interactions and interventions accurately and in compliance with policies and procedures.
Requirements
- Masters Degree in Social Work
- Montana Clinical Social Worker License
- 3 years of experience with adult populations and familiarity with broad range of psychiatric disorders
Preferred Qualifications
- Within 5 years of hire: OSW-C certification (for the Cancer Center LCSW)
- Prior oncology experience preferred for the LCSW in the Cancer Center

Pay
The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. The base pay range is $33.52 - $52.03 per hour.
Schedule
This position is full-time and works on a day shift.
