LCSW Social Worker

Length of assignment: 13 weeks
Shift / Hours per week: 8a-4:30p with weekends on request

Position Summary

Coordinates patient care throughout the rehabilitation stay, facilitates interdisciplinary treatment planning, manages utilization review and discharge planning, and ensures patients have the resources and services needed for a safe and successful transition to the next level of care.

Minimum Required Qualifications

• LCSW – active or pending CA licensure
• BLS
• 2 years recent social work experience in a hospital setting
• 3-5 years clinical experience, including med-surg and post-acute care settings
• Knowledge of reimbursement benefits and requirements including but not limited to Medicare, Medicaid and managed care
