LCSW Social Worker

Position Summary

Provides psychosocial assessment, counseling, care coordination, discharge planning, and resource referrals to help patients and families navigate medical, emotional, social, and financial challenges during hospitalization and throughout their continuum of care.

Minimum Required Qualifications

• MA LCSW or LICSW
• BLS
• 2 years acute care medical social worker experience (not psych or behavioral health)

Schedule

• 13-week assignment
• Shift: 7:00 a.m. – 3:30 p.m.
• Start date: September 8, 2026

Pay Rate

$1,600.00 – $2,500.00
