Primary Responsibilities
- Conduct psychosocial assessments and develop practical, person-centered service plans
- Provide supportive counseling and crisis intervention when needed
- Help coordinate care with hospitals, providers, and community partners
- Support individuals through transitions, including hospital admissions and discharges
- Accompany guests and residents to appointments and advocate on their behalf
- Facilitate groups based on program needs
- Maintain clear, timely documentation to support care and program compliance
- Collaborate with staff across programs and serve as a mental health resource
- Participate in the on-call rotation
Requirements
- Master’s Degree in Social Work (or related field), LCSW required.
- At least 2 years of experience working with individuals living with mental illness and experience working with individuals experiencing homelessness.
- Ability to build trust and work effectively with a diverse and often complex population
- Strong communication, collaboration, and problem-solving skills
- Valid NYS Driver’s License and ability to transport clients as needed
