Job Summary and Qualifications
This position supports the mission of Swedish Medical Center by optimizing the quality of life for all patients served. The Social Worker III-Case Manager assesses psychological, social, cultural, and financial needs impacting the health and recovery of high-risk patients. They coordinate services through an interdisciplinary process, ensuring appropriate levels of care and discharge planning.
- Utilize social services, counseling, and clinical knowledge in assessments.
- Coordinate services through an interdisciplinary process.
- Assess appropriateness of admission, continued hospitalization, and appropriate level of care.
- Discharge planning begins at admission and reassessed throughout the course of hospitalization.
- Knowledge of payer source implications on discharge plan, resources, and placement options.
Qualifications and Position Requirements
- Licensed Clinical Social Worker (LCSW) in the state of Colorado.
- Specialty certification in case management preferred.
- BLS certification through American Heart Association.
- Master of Social Work (MSW) from an accredited school of social work.
- 3-5 years recent clinical social work experience with acute care hospital settings and case management.
- Excellent written, verbal, and interpersonal communication skills.
- Knowledgeable of State and JCAHO standards.
- Knowledgeable of national criteria for medical necessity for hospital admissions and continued stays.
- Able to perform multiple tasks simultaneously and work in high-stress environments.
- Able to apply critical thinking and formulate diagnostic impressions.
- Able to provide crisis intervention and utilize system resources/referrals.

Pay and Schedule
Routine workdays and hours are Monday-Friday, 8 am-4:30 pm with a minimum of one (1) weekend/month required for PRN staff.
