Social Worker II - Social Work Services

Responsibilities

• Provide emotional support, crisis intervention, and counseling for Pediatrics, Adolescents, Adults, Older Adults, and their significant others to help cope with social and emotional challenges.
• Offer information, referrals, and assistance for financial aid and supportive services related to hospitalization, illness, disability, or trauma.
• Conduct case management and milieu management, complete psychosocial assessments, and assist in developing and re-evaluating Treatment Plans.
• Work as part of a multidisciplinary team and coordinate discharge planning.
• Complete necessary documentation and maintain clinical records for each client throughout their stay.

Qualifications

• Master’s Degree in Social Work or related field OR Master’s Degree in Marriage and Family Therapy or related field.
• In states without required licensure, current registration with State Board is required within 3 months of hire. In states with required licensure, license must be obtained at time of hire and maintained (LSW or higher required).
• Minimum one (1) year of hospital or outpatient experience with emphasis in mental health care.
• Experience working with a diverse population addressing a wide range of issues (e.g., death and dying, maternal and child care, substance abuse, disability, chronic illness).
• Current state license required upon hire and must be maintained.

Pay

A reasonable compensation estimate for this role is $35.58 per hour. The exact starting compensation will be determined based on factors such as skillset, years of applicable experience, education, credentials, and licensure.
