Social Worker II
About the role
The Social Worker at Parkview Regional Medical Center assesses patient/family needs and coping skills that support the patient’s unique cultural and spiritual beliefs. They provide holistic patient/family centered care, education, interventions, and community resources specific to the patient/family need. They also provide crisis intervention, complex or long-term placement linkage to appropriate community resources, participate in discharge planning and treatment planning, and coordinate with the care team and other coworkers to facilitate the patient's goals and achievement of quality and cost-effective outcomes.
Responsibilities
- Affirmatively assess patient/family needs and coping skills that support the patient’s unique cultural and spiritual beliefs.
- Provide holistic patient/family centered care.
- Provide education, interventions, and community resources specific to the patient/family need.
- Provide crisis intervention, complex or long-term placement linkage to appropriate community resources.
- Participate in discharge planning and treatment planning.
- Collaborate with the care team and other coworkers to facilitate the patient's goals and achievement of quality and cost-effective outcomes.
Requirements
- Masters Degree in Social Work required.
- Current Indiana LSW or LCSW license.
- Minimum of 2 years professional experience working with patients/families with social complexities and cultural barriers to healthcare.
- Prior discharge planning or medical social work experience preferred.
- Previous work experience in the hospital setting preferred.
- Thorough working knowledge of social work process, community agencies, services, and resources available to assist patients and families in the resolution of emotional, social, and financial difficulties related to change in patient's health status.
- Sound analytical and organizational skill to assess patient's needs, plan and coordinate appropriate therapeutic intervention, and manage case load to achieve efficiency and productivity.
- Good verbal and written communication skills.
