About the Role
The Social Worker facilitates patient care through resource coordination, ensuring optimal health, appropriate care access, and patient self-determination. They assess patient transition needs, identify at-risk individuals, and conduct psycho-social assessments to promote timely discharge and prevent readmissions.
Responsibilities
- Perform complex psycho-social assessments and interventions.
- Assist with cases involving adoption, abuse, and neglect, including referrals to agencies.
- Oversee and implement transition plans, leading multidisciplinary care conferences.
- Communicate care plans with patients, families, and healthcare teams, ensuring post-acute needs are met.
- Ensure accurate documentation in case management systems and maintain patient information.
- Lead population care efforts, ensure regulatory compliance, and provide education to stakeholders.
- Perform related duties as needed to support patient care and team performance.
Requirements
- Master's of Social Work (MSW) and active licensure/certification.
- Preferred: 2+ years of acute hospital experience and Accredited Case Manager (ACM) credential.
