About the role

The Social Worker facilitates patient care through resource coordination, ensuring optimal health outcomes, appropriate resource utilization, and respecting patient self-determination.

Responsibilities

• Conducts complex psycho-social assessments and interventions.
• Manages transition planning and assists with cases involving adoption, abuse, and neglect.
• Leads multidisciplinary care conferences.
• Develops and oversees transition plans.
• Communicates effectively with patients, families, and healthcare team members.
• Ensures accurate documentation and compliance with regulatory standards.
• Provides education to stakeholders.
• Supports post-acute care needs and facilitates referrals.
• Ensures care aligns with regulatory and accreditation standards.
• Performs related duties as assigned to enhance patient care and team performance.

Requirements

• Master's of Social Work (MSW).
• Active licensure or certification to practice.
• Preferred: two years of experience in acute hospital settings.
• Preferred: accreditation as an Accredited Case Manager (ACM).
