About the role

Crouse’s Care Coordination Services team is hiring a Social Worker to work collaboratively with Care Managers and other members of the multidisciplinary healthcare team to ensure the appropriate allocation of hospital resources while maintaining the quality of care.

Responsibilities

• Participates in the development and implementation of a comprehensive integrated discharge plan.
• Identifies patient and family psychosocial and environmental needs related to admission, diagnosis, treatment, and discharge.
• Actively participates in the discharge process following federal and state regulations and accreditation standards, managing care across the continuum to ensure a safe discharge within a timely manner.
• Provides crisis interventions to patients and families as needed.
• Demonstrates age-appropriate communication in assigned work area.
• Interacts with physicians, members of the multidisciplinary healthcare team, patients, families, and community agencies to achieve optimal patient outcomes.

Requirements

• Master’s Degree in Social Work required.
• LMSW license preferred.
• 2 to 3 years of Social Work experience required.
• Case Management, Discharge Planning, or home care experience preferred.
• Excellent oral and written communication and interpersonal skills.
• Knowledge of federal and state regulations (DOH, Medicare/Medicaid).
• Strong organizational and problem-solving skills.
• Knowledge of third-party payers and/or managed care principles.

Pay

$60,000.00 - $65,905.00 per year.

Schedule

Full-time, day shift, 8:00 AM – 4:30 PM.
