About the role
EvergreenHealth Hospice is an interdisciplinary team dedicated to providing the highest quality of life possible for patients and their families. Collaboration among physicians, nurses, social workers, certified nursing assistants, spiritual care providers, pharmacists, and trained volunteers ensures comprehensive symptom management and support during end-of-life care.
Responsibilities
- Initiate, develop, implement, and revise the patient’s plan of care in coordination with the multidisciplinary home health team, the patient’s primary physician, family members, and community resources.
- Provide education to patients, families, and caregivers on knowledge deficits or needs related to the patient’s condition.
- Assist patients, families, and caregivers with accessing in-home help, community resources, supportive counseling, long-term planning, and community education.
- Maintain current knowledge of community resources to address social, financial, medical, and spiritual needs of patients and families.
- Perform patient assessments including psychosocial, functional, mental status, and lethality assessments.
- Provide therapeutic interventions and instruction in appropriate psychotherapy and life skills such as assertiveness, coping, grieving process, problem-solving, communication, stress management, suicide precautions, behavioral modification, and impulse control.
- Adapt communication to the educational level and learning style of patients, families, and caregivers, and effectively engage with diverse cultures and lifestyles.
Requirements
- Master’s Degree in Social Work (MSW).
- One of the following credentials is required:
- Licensed Independent Clinical Social Worker (LICSW)
- Licensed Advanced Social Worker (LASW)
- Credentialed as an Agency Affiliated Counselor (AAC)
- Social Worker Associate License (LSWAA or LSWAIC)
- If hired prior to January 2012, one of the following may also be accepted:
- Licensed Mental Health Counselor (LMHC)
- Licensed Mental Health Counselor Associate (LMHCA)
- Current Washington State Driver’s license and proof of insurability.
- One year of social work experience in a related healthcare setting.
- Current Healthcare Provider BLS certification by date of hire.
- Map reading skills and ability to drive in varied situations and inclement weather.
Preferred Qualifications
- Previous hospice or home health care experience.

This position is covered by a collective bargaining agreement between EvergreenHealth and SEIU Healthcare 1199NW. Union membership is optional.
