The ideal candidate will have experience working with hospice, home health, dementia care, and developmental disabilities, and more. The Social Worker will be responsible for providing care and support to patients and their families, as well as conducting research and developing systems to improve patient outcomes. Qualifications Required Master's degree in Social Work or related fieldExperience working with adults and elderly populationsDriver's license and car insuranceStrong communication skillsAbility to work independently as well as part of a teamLCSW or LMSW License What You’ll Do Assesses the psychosocial status of patients and families/caregivers related to the patient's medical condition and environment and communicates findings to the registered nurse and other members of the interdisciplinary groupConducts assessments of patients' needs and develop care plansProvides individual and group counseling to patients and their familiesCollaborates with healthcare professionals to ensure the best possible care for patientsProvides information and referral services for patients and families/caregivers regarding practical and environmental needsProvides information to patients and families/caregivers and community agenciesServes as a liaison between patients and families/caregivers and community agenciesDevelops systems to improve patient outcomesConducts research to identify best practices in social workProvides education and training to staff on social work topicsMaintains accurate records of patient care
