Qualifications
- Social Work Degree MSW required
- Current State Licensure in Social Work
- Minimum of one year’s experience in health care or hospice setting
- Valid driver’s license and proof of insurance is required
HOURS
Monday-Friday 8:30AM-5PM
On-Call 3 Shift/Month

What You’ll Do
- Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient’s plan of care.
- Assists in providing an assessment in the patient’s identified residence and assistance when this is not safe, and another plan is required.
- Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers.
- Aids team members as needed to ensure consistent provision of this level of service.
- Counsels patient and family/caregivers and staff as needed in relationship to stress, and other identified coping difficulties.
- Accurately and timely documents patient services to reflect the plan of care.
- Assists with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver.
- Obtains resources from community social, health, pharmaceutical, and welfare agencies to assist in meeting patient/family needs.
- Serves as liaison between patients and families/caregivers and community agencies.
Pay
Salary Range: $62,000-$80,000 - Offer based on experience
