Responsibilities
- Conduct face to face resident interviews and assessments of resident cognitive, mood, behavioral status in addition to other psychosocial factors.
- Participate in planning for resident discharges.
- Available to respond to clinical emergencies, regulatory agency visits, surveys and issues outside of normal work hours as needed.
- Direct and supervise the delivery of medically related social services to all residents of the facility, as well as their family members and significant others as needed.
- As needed, participate in the clinical evaluation and screening of potential admissions to the facility.
- Work in collaboration with the facility interdisciplinary treatment team in accessing information regarding, and as appropriate making referral to, community based services for residents who wish to access such services.
- Serve as a facility treatment team liaison with facility contracted mental health practitioners and consultants.
- Actively participates in developing and implementing new behavioral and psycho social service programs and the presentation of such programs to physicians, family members, and residents.
Qualifications
- Bachelor’s Degree in Social work or related human services field, required.
- A minimum of one years’ experience in long term care providing direct social services, preferred.
- Spanish speaking preferred.
