Social Worker - Hospice

Job Summary

At Brighton/Suncrest Hospice, we aim to enhance hospice care in your area by improving staff-to-patient ratios, leading to more frequent clinical visits and reduced clinician caseloads. We are proud to be CHAP certified.

Responsibilities

• Assesses, diagnoses, and treats patient's mental and social conditions
• Counsels individuals and/or families
• Updates case records
• Advises agency personnel, clients, and families about available community resources
• Develops and updates client's plan of care
• Reports client's response to treatment and condition changes to physicians
• Maintains client care records and communicates with relevant personnel
• Attends and participates in client care conferences, staff meetings, and agency committee meetings

Qualifications

• Thorough knowledge of social work principles and methods
• Knowledge of individuals and group behavior
• Ability to work independently and effectively with clients, families, staff, and external agencies
• Excellent oral and written communication skills
• Master’s degree in Social Work required
