About the Role

AuthoraCare Collective is seeking a part-time Home Health Social Worker (24 hours per week) with a flexible schedule, based in High Point. This role provides psychosocial support and counseling to patients and their families, helping them navigate emotional, social, and environmental challenges related to illness, disability, and recovery.

The Social Worker assesses patients’ social and emotional needs, develops individualized care plans, and connects patients with community resources to promote optimal health outcomes and independence at home as part of an interdisciplinary care team.

Responsibilities

  • Completes comprehensive psychosocial assessments, providing counseling and crisis intervention.
  • Assists with advance care planning and connects patients to community resources, including financial, housing, and transportation support.
  • Works in collaboration with the interdisciplinary team and home health director.
  • Develops and carries out individualized plans of care related to the social determinants of health.
  • Identifies appropriate resources to meet patient, caregiver, and family needs.
  • Serves as an advocate for patients and families.
  • Actively participates in the interdisciplinary group (IDT) with a focus on collaboration and problem-solving.
  • Provides effective reports to the IDT based on regulatory requirements for patient care planning and evaluation.
  • Maintains effective relationships and partnerships with community healthcare providers, agencies, and senior services.
  • Completes required documentation according to agency accreditation standards, ensuring notes and patient records are complete, accurate, current, and clear.
  • Documents consultation and/or referrals to Home Health leaders for clinically complex cases requiring advanced clinical interventions.

Requirements

  • Master’s degree in social work.
  • One year of social work experience in a mental health or medical setting.
  • Ability to travel to, enter, and function in the homes of any patient/caregiver within the service area.
  • Valid state-issued driver's license, automobile liability insurance at agency-required limits, and own transportation.
