About the role

The Hospice Social Worker provides a wide variety of services to patients/caregivers/families in accordance with the individualized plan of care. The Social Worker actively participates in coordination of all aspects of the patient’s care in accordance with current professional standards and practice. The Social Worker’s expertise lies in identifying and addressing the emotional and biopsychosocial needs in advanced illness at end of life.

Responsibilities

• Provide a wide variety of services to patients/caregivers/families in accordance with the individualized plan of care.
• Participate in coordination of all aspects of the patient’s care in accordance with current professional standards and practice.
• Identify and address the emotional and biopsychosocial needs in advanced illness at end of life.
• Perform supervisory duties when recommended and educated to do so by organizational leadership.

Requirements

• At least 2 years of Master's level Social Work experience in an interdisciplinary or health care setting.
• A minimum Master’s degree in Social Work (MSW) from a Social Work accredited school.
• Appropriate licensure from the governing body for practice from the applicable state: District of Columbia, State of Maryland, and/or the Commonwealth of Virginia.

Pay

This position pays a range of $62,249.35 - $92,651.93. Consent to drug and/or alcohol testing after a conditional offer of employment is made, as well as on-going compliance with the Drug-Free Workplace Policy.
