Social Worker - Grade 1 & 2 ABI Unit

About the role

This is a fixed-term parental leave position until February 2027, located at Caulfield Hospital in the ABI Rehabilitation Centre. The role is full-time (80 hours per fortnight) with flexible monthly ADO, classified as Social Worker Grade 1-2.

You will assist Bayside Health - Alfred Care Group in supporting patients with an Acquired Brain Injury. The Social Worker is responsible for identifying and meeting the needs of this specialist patient group in collaboration with clinical units, particularly managing complex clinical cases.

Key aspects of the role include:

• Providing advanced casework expertise and consulting with staff across disciplines on patient care issues.
• Developing and maintaining community linkages and collaborating with internal and external stakeholders.
• Consulting with NDIS, TAC, Workcover, ACAS, Transitional Care Program (TCP), Carer Services, Office of the Public Advocate (OPA), and Victorian Civil Administrative Tribunal (VCAT).

The Department

The Social Work Department operates in accordance with the philosophy and goals of Bayside Health, focusing on quality of life and participation in care. The department aims to address the psychosocial impacts of illness and hospitalisation for patients and their families, facilitating a smooth transition from hospital and rehabilitation to the community.

Responsibilities

• Identify and address the psychosocial needs of patients with an Acquired Brain Injury and their families.
• Work collaboratively within a multi-disciplinary team to manage complex clinical cases.
• Demonstrate excellent interpersonal, negotiation, and communication skills (written and verbal).
• Display flexibility in social work practice and contribute to an inter-disciplinary team environment.
• Consult with various funding bodies and services (e.g., NDIS, TAC, Workcover) to support patient care.
• Maintain and develop community linkages with internal and external stakeholders.

Requirements

• Eligibility for membership of the Australian Association of Social Workers.
• Bachelor of Social Work or recognised equivalent.
• Experience in an acute or sub-acute hospital setting is preferable.
• Ability to work effectively in a multi-disciplinary team and with families, exhibiting initiative and facilitative qualities.
• Knowledge of NDIS, Aged Care services, and TAC funding models is advantageous.
• Proficiency in working within an Electronic Medical Record system.
