Job Responsibilities
- Participate in planning related to resident, family, and community interests.
- Participate in discharge planning, development and implementation of social care plans and resident assessments.
- Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, arranging transportation, etc., as necessary, coordinating activities with other departments as necessary.
- Promotes optimum transition of resident for appropriate and safe placement for discharge.
- Provides ongoing communication with resident and responsible persons on progress/status during stay.
- Provide information to the resident/family on Medicare/Medicaid as well as other financial assistance programs available.
- Provide consultation to staff, community agencies, etc., in efforts to solve resident concerns through the development of social service programs.

Minimum Qualifications
- Must possess a bachelor's or master's degree in social work or a bachelor's or master's degree in a human services field (i.e. sociology, special education, rehabilitation counseling and psychology.)
- One year of supervised social work experience in a healthcare setting working directly with individuals.
- Experience in long-term care, working with the senior population and dementia counseling.
- Must be licensed as a social worker in Michigan.
- Must lead by example and possess superior customer service skills and professionalism.
- Flexibility to adapt to ongoing change and work in a fast-paced, customer driven environment.
