Job Summary

Aid patients with complex psychosocial needs, assist with eligibility for social programs and funding sources, provide short-term counseling, and coordinate discharge plans.

About the Role

Work alongside the RN Case Manager to manage socially complex patients identified through initial assessment and referral.

Responsibilities

• Aid patients with complex psychosocial needs
• Assist with eligibility for social programs and funding sources
• Provide short-term counseling and crisis intervention
• Cookordinate and facilitate the development of a safe discharge plan of care for high-risk patient populations

Requirements

• Graduate from an accredited Social Work Program with Bachelors in Social Work
• Preferred: MSW
• Required: Clinical rotation experience (can be taken into consideration)
• Preferred: Experience in hospital social work
• Knowledge of managed care practices, community resources, regulatory and insurance reimbursement issues preferred

Department Position Summary

The Case Manager Social Worker works in conjunction with the RN Case Manager to manage socially complex patients identified by initial assessment and referral. The Social Worker identifies the optimal resources available to meet the ongoing needs of the patients and their caregivers along the care continuum. The social worker is required to initiate this plan on admission.
