Social Worker - Full time/Days/Mon-Fri/Occasional weekend to support admissions

Principle Duties and Responsibilities

• Responsible for exercising discretion and independent judgment to assist the management of the Social Services Department
• Encourage residents through counseling and other interventions to achieve their highest level of psychosocial interaction
• Implement person-centered care plans for residents and monitor and follow-up on directives as outlined in the resident's plan of care
• Counsel residents/families regarding psychosocial, adjustment, and behavioral issues
• Participate in Behavior Management Team meetings to monitor resident behaviors and psychotropic medication use
• Participate in interdisciplinary care conferences with residents and families
• Aid with resident discharge planning
• Perform other duties as required.

Qualifications

• Ability to give instructions, organize time and complete tasks with minimal supervision
• Ability to manage multiple requests and projects simultaneously
• Excellent written, verbal and interpersonal skills necessary

Experience

• A broad knowledge of social work skills and the ability to apply necessary skills and techniques in a medical setting is preferred
• Long-term care social work experience preferred
• Experience working with PointClickCare software a plus

Education

• Masters in Social Work required
