Social Services/Admissions, Full-time

About the role

You will assist in the planning, organizing, implementing and evaluation of the overall operation of our Facility's Social Services department. You will ensure that the medically related emotional and social needs of the residents are met and maintained on an individual basis in accordance with current federal, state and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Administrator.

Responsibilities

• Aid in the development and implementation of social services programs
• Evaluate resident needs and develop appropriate care plans
• Coordinate with other departments to ensure comprehensive care
• Ensure compliance with regulatory and compliance standards
• Document resident progress and communicate with family members
• Provide emotional support to residents and their families

Requirements

• Bachelor's Degree in Social Work, Licensed Social Worker preferred
• 2 years of experience in Social Services in Long Term Care
• Intermediate to advanced Microsoft Office Suite Package (Word, Outlook, Excel, PowerPoint)
• Regulatory and Compliance Knowledge
