Social Worker (Fee For Service)

Role Overview This position involves providing psychosocial support and social work services to hospice patients and their families across inpatient, home care, and nursing home settings. The social worker will serve as a member of the hospice interdisciplinary team (IDG) and act as a liaison between hospice staff and inpatient or nursing home staff. Key responsibilities include developing social work plans of care, setting goals, and making recommendations based on patient and family needs. The role also entails creating discharge plans when appropriate. Qualifications And Skills Graduate from an accredited School of Social Work.Minimum of one year of healthcare social work experience.Knowledge of Word and Excel preferred.Valid NYS MSW license.Valid NYS driver’s license and access to a vehicle.Ability to work within an interdisciplinary team environment.Proficient in inputting clinical data.
