Social Worker- Days

Part-time, Day shift position located in Germantown. Reports to the Manager of Care Coordination.

About the role

In collaboration with Care Coordination staff, this position is directly involved in the process of assessment, planning, facilitation, and advocacy for options and services to meet individual patient’s health care needs. Through communication and identification of available resources, participates in the promotion of quality outcomes, patient satisfaction, and cost-effective care delivery.

Responsibilities

• Recognizes special needs/concerns of populations served and adjusts communication accordingly (e.g., language barriers, diminished hearing, need for concrete terms).
• Collaborates with Care Coordination staff and multidisciplinary team members regarding discharge planning and arranges for needed care in the home or an alternate setting.
• Provides support and counseling to patients/families experiencing and/or anticipating issues adjusting to illness, catastrophic diagnoses, changes in living situations, and bereavement; establishes a plan.
• Documents interventions and outcomes as required in the electronic record.
• Actively participates and provides pertinent information in multidisciplinary rounds on assigned patients.
• Completes appropriate assessments for assigned patients to identify gaps in care, develop goals, and facilitate interventions and resources.
• Performs diversified duties with time limitations with a high degree of accuracy.
• Works independently and as part of a team.
• Demonstrates problem-solving, critical thinking, and priority-setting skills.

Qualifications

• Master's Degree in Social Work/Services.
• At least one year of social work experience in an acute-care setting (MSW internship experience is acceptable).
• State of Maryland LGSW, LCSW, or LCSW-C licensure.
• Demonstrated ability to work with all age groups of patients.
• Able to communicate effectively, demonstrating good audio and visual acuity.
• Ability to prioritize tasks and manage time effectively.
