Social Worker, Days

Responsibilities

• Collaborates directly with the care team, care management team and patient, and/or family, and/or the patient’s representative to assess the patient’s continuum of care needs and ensure the patients transition to the least restrictive care environment.
• Assesses the patient’s psychosocial and continuum of care needs.
• Interventions may include, but are not limited to, family support, crisis management, consultations with a staff physicians, Adult or Child Protective Services reporting/coordination, custody/guardianship issue coordination, indigent patient program coordination, substance-use support coordination, intimate partner violence support, bereavement.
• Interventions may include referrals and coordination of acute rehabilitation, extended care, durable medical equipment, home health, and other resources as necessary.

Qualifications

• One year social work or One year social work internship.
• Must have hospital and/or payer credentialing completed before functioning in this role.
• Master Degree in Social Work from accredited programKentucky - Certified Social Worker (CSW), or Indiana - Licensed Social Worker (LSW)
• Desired: Licensed Clinical Social Worker (LCSW)
