Social Worker - Ambulatory Care Network - Days
About the role
Our Audubon Practice, part of NewYork-Presbyterian’s Ambulatory Care Network, provides triage, screening, referral, and crisis intervention to ensure the mental well-being of our patients. You will see patients both virtually and in person through scheduled appointments and walk-ins, handling responsibilities such as crisis intervention, safety assessments using evidence-based screening, concrete services, home care assistance and follow-up, psychosocial assessments, and referrals to community-based organizations. Collaboration with interdisciplinary teams and across the network of ambulatory care clinics is essential.
Responsibilities
- Provide crisis intervention and assess patient safety using evidence-based screening tools
- Deliver concrete services and assist with home care coordination and follow-up
- Conduct psychosocial assessments
- Make referrals to community-based organizations (CBOs)
- Work collaboratively with interdisciplinary teams and across ambulatory care clinics
- See patients both virtually and in person via scheduled appointments and walk-ins
Requirements
- Master’s Degree in Social Work (MSW)
- New York State License as a Licensed Master Social Worker (LMSW)
Preferred Qualifications
- Bilingual (Spanish)
- Experience in a busy outpatient setting
- Experience working with interdisciplinary teams
Schedule
Monday – Friday, 9:00 AM – 5:00 PM (except Thursdays, 9:30 AM – 5:30 PM).
Pay
Salary range: $43.86 – $58.73 per hour.
