About the role
The Mount Sinai Hospital is a 1,134-bed facility with an extensive outpatient and specialty care network. It offers comprehensive social work services and programming to meet the needs of diverse populations.
Responsibilities
- Complete full psychosocial assessments and work closely with the CTMS interdisciplinary team including the nurse coordinator, eligibility assistant, mental health professionals, primary care providers, surgeons, medical specialists, and outside service providers to support trans patients through their medical transitions.
- Serve a pivotal role in facilitating appropriate medical interventions when required. Surgical support includes confirming mental health evaluations, necessary letters, pre-surgical treatments, and ensuring post-surgical care resources are in place.
- Develop and maintain support groups for CTMS patients, including pre- and post-surgical patients, provide short-term therapy, and participate in CTMS team meetings, steering committee meetings, and community advisory board meetings.
- Be responsible for administrative duties such as creating and organizing patient lists with up-to-date surgical information, co-leading case conference meetings, and mediating between in-patient and out-patient teams.
Qualifications
- Experience working with LGBTQIA+ individuals with specific familiarity with transgender individuals.
- Excellent engagement, organizational, collaborative, and clinical skills.
- Excellent administrative and time-management skills.
- A collaborative and flexible attitude to meet the changing needs of the interdisciplinary team.
- Bilingual (English/Spanish) preferred.
