Social Worker - Case Manager Full Time / Day Shift

About the role

We are seeking a full-time Social Worker - Case Manager to join an interdisciplinary team, focusing on assessment, care coordination, and discharge planning for patients across all age groups. This role emphasizes patient advocacy, risk assessment, and effective discharge planning to improve patient outcomes and reduce readmissions.

Responsibilities

• Assess patients' discharge needs within 24 hours of admission and throughout the stay
• Participate in multidisciplinary meetings to address discharge barriers
• Educate patients and families on discharge plans and post-acute care
• Evaluate social determinants of health and risk factors impacting care
• Secure authorization for post-acute services and document all interactions
• Support ethical practices, patient rights, and compliance with regulations
• Provide psychosocial assessments and interventions for high-risk populations
• Connect patients with community resources and ensure safe transitions of care
• Mentor and guide unlicensed team members as needed

Requirements

• Master’s degree in Social Work
• LICSW or LCSW licensure
• At least three years of social work experience, preferably in acute care
• Current BLS certification

Qualifications

• Case management certification (preferred)
• Experience in hospital settings (preferred)
