Social Worker - Case Manager Full Time / Day Shift

About the role

Join an interdisciplinary team as a Social Worker - Case Manager, responsible for assessing, planning, advocating, and coordinating patient care from admission to discharge while ensuring compliance with the Case Management Model.

Responsibilities

• Conduct discharge planning assessments within 24 hours of admission.
• Participate in multidisciplinary meetings.
• Educate patients and families about discharge plans, considering social determinants and payor factors.
• Obtain authorizations for post-acute services.
• Maintain thorough documentation and ensure adherence to regulatory standards.
• Provide psychosocial assessments and advocate for at-risk populations.
• Coordinate community resources and support advanced directives.
• Mentor team members.

Qualifications

• Master’s degree in Social Work.
• LICSW or LCSW licensure.
• At least three years of social work experience.
• BLS certification.

Preferred Qualifications

• Hospital experience.
• Case management certification.
