Social Worker - Case Manager Full Time / Day Shift

About the role

Join an interdisciplinary team as a full-time Social Worker - Case Manager, responsible for assessing, planning, advocating, and coordinating patient care from admission to discharge in a hospital setting.

Responsibilities

• Conduct discharge planning assessments within 24 hours of admission and throughout hospitalization.
• Participate in multidisciplinary meetings to identify and resolve discharge barriers.
• Educate patients and families on discharge plans and post-acute needs, considering social determinants of health and payor factors.
• Secure authorization for post-acute services and ensure timely documentation of interactions and interventions.
• Support compliance with regulations and ethical standards, including HIPAA and CMS requirements.
• Provide psychosocial assessments, advocate for at-risk populations, and coordinate community resources for safe transitions of care.
• Delegate tasks to unlicensed team members and serve as a resource and mentor to colleagues.

Qualifications

• Master’s degree in Social Work.
• LICSW or LCSW licensure.
• Minimum of three years of social work experience, with preferred experience in acute care.
• Current BLS certification.
