Social Worker Case Management

About the role

Deliver direct patient care by administering physician and nursing orders, implementing care plans, and following established organizational policies. Monitor and document patient conditions, continuously updating the healthcare team on changes and response to treatment. Coordinate multidisciplinary care and manage patient discharge planning in collaboration with the healthcare team.

Requirements

• Licensure / Certification / Registration: Clinical Counselor, Social Worker, Therapist specializing in Marriage & Family
• Education: High school diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience

Qualifications

• Experience in healthcare administration or related field
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Knowledge of healthcare regulations and standards
