Your future role at a glance Location: Elk Grove Village, ILFacility: Ascension Alexian BrothersDepartment: Case ManagementSchedule: Day shift | Full-time How you’ll make an impact in this role Develops care plans for patients based on assessment findings and consideration of needs and preferences.Assesses complex emotional, psychological and physical issues to understand what the patient requires and how social work can provide support.Advocates for patients' needs and ensures the medical system respects their rights and caters to their preferences.Maintains ongoing dialog with leadership and care transition team members to ensure effective implementation and reevaluation of care plan. What Minimum Requirements You’ll Need Licensure / Certification / Registration: Required Credential(s):None Required.Preferred Credential(s):Case Manager credentialed from the Commission for Case Manager Certification (CCMC). ACM also accepted.Certified Social Worker. Education Bachelor's degree in Social Work required. Work Experience 3 years of experience is preferred.
