Minimum Requirements
- Licensure / Certification / Registration: Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted.
- Certified Social Worker preferred.
- Education: Bachelor's degree in Social Work required.
- Work Experience: 3 years of experience preferred.
Additional Preferences
- Must have Social Worker license.
- Masters degree in Social Work required.

What You'll Make an Impact On
- Assess complex emotional and social barriers to develop personalized care plans that address each patient’s unique needs and preferences.

- Advocate for patient rights and preferences within the medical system to ensure the care provided is respectful and person-centered.
