Social Worker Care Companion

About the role

This part-time position is ideal for professionals who want to make a direct impact on the lives of older adults—helping them feel heard, supported, and emotionally connected within their environment.

Key Responsibilities

• Visit designated facilities in accordance with the schedule set with your supervisor
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs
• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns
• Document visit notes and maintain detailed records of interactions and interventions after each visit
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP

Qualifications

• Associates, Bachelor’s or Master’s degree in Social Work, Psychology, Counseling, or another mental health–related field
• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings
• Strong communication, documentation, and relationship-building skills
• Minimum 1 year of experience

Pay

Flat salary of $14,000 - $16,000 annually

Schedule

Approximately 10 hours per week, Monday-Friday, within regular 8-5 hours
