About the role
We are seeking a compassionate and dedicated Social Worker/Care Companion to join our Collaborative Care team at Carease Health - MD. As a valued member of our team, you will play a vital role in providing emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities.
Key Responsibilities
- Visit designated facilities in accordance with the schedule set with your supervisor
- Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs
- Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns
- Document visit notes and maintain detailed records of interactions and interventions after each visit
- Collaborate with psychiatric professionals to ensure comprehensive care
Qualifications
- Associates, Bachelor’s or Master’s degree in Social Work, Psychology, Counseling, or another mental health–related field
- Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings
- Strong communication, documentation, and relationship-building skills
