About the role
At Carease Health, we are dedicated to providing comprehensive preventative care programs that prioritize the physical and mental well-being of our patients. As a Social Worker/Care Companion, you'll play a vital role in our Collaborative Care team, providing emotional support and mental health assessments to residents in skilled nursing facilities.
Key Responsibilities
- Visit designated facilities in accordance with the schedule set with your supervisor
- Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs
- Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns
- Document visit notes and maintain detailed records of interactions and interventions after each visit
- Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP
Qualifications
- Associate, Bachelor’s, or Master’s degree in Social Work, Psychology, Counseling, or another mental health–related field
- Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings
- Strong communication, documentation, and relationship-building skills
