About the Role
Assesses patients and families and provides intervention and treatment to support patients.
Addresses psychosocial needs and patterns of adjustment throughout the hospitalization and collaborates with the multidisciplinary team to formulate treatment plans to maximize emotional, social and physical well-being.
Provides information and education regarding health care and community resources.
