Social Worker BSW

We are seeking a Social Worker BSW to provide comprehensive, patient-centered care management services in an acute care setting. Key Responsibilities Include Care coordination and discharge planning to ensure seamless patient transitions.Documentation of interventions, regulatory compliance, and patient advocacy.Identifying and addressing social, financial, cognitive/behavioral, or legal barriers to care access for vulnerable patients. Qualifications Bachelor's Degree in Social Work from an accredited program.Alaska Baccalaureate Social Worker License upon hire.At least 1 year of social work experience in healthcare, preferably in medical or long-term care settings. Preferred Skills And Experience National certification in a specialty area.Experience in acute care or case management programs.Knowledge of healthcare terminology and prior case management experience.
