Social Worker

About the role

The Social Worker position at NHC focuses on supporting individual patients and families through adjustments to center life and addressing crises such as illness, disability, and death. Key responsibilities include advocating for patients and their families, coordinating discharge planning, and participating in developing comprehensive patient care plans.

Responsibilities

• Serving in an advocacy role for patients and their families in expressing, defining, and resolving grievances
• Coordinating discharge planning functions
• Reporting to the Director of Social Work Services on the current status of social services and identified needs
• Maintaining confidentiality of professional information
• Participating in the development of patient care plans in collaboration with other healthcare professionals

Requirements

• Holds a Bachelor's or Master's degree in social work from an accredited program
• Knowledge of the case management process and needs of geriatric patients, including understanding funding resources

Qualifications

• Proven experience in social work
• Strong communication and interpersonal skills
• Ability to maintain confidentiality and handle sensitive information
