About the role
The Social Worker position performs professional and technical duties involved in assisting individuals, families and/or groups of people by providing them with services designed to help them meet a variety of human, personal and environmental needs. Performs related duties as required.
Responsibilities
- Perform concise, comprehensive social assessments and developing service plans based on the customers' individual needs.
- Provide information and referral services to customers and the public that enables them to utilize the programs' services to best address their needs.
- Provide short-term or long-term case management services to all populations served.
- Conduct home visits to customers' homes and/or other agencies/locations within the community and/or designated co-located partnership sites.
- Research and advocate for customers to receive appropriate services through community resources such as public assistance programs, drug rehabilitation programs, and housing shelters.
- Care for individuals with mental illness or developmental disabilities.
- Work with various established Metro partnerships as well as being flexible to work across the Division of Programs in any of the agency identified program units.
- May provide written summaries and any reports from completed interviews from the assigned Conservatorship cases.
- Build and maintain working relationships with various Metro Departments and other community agencies to ensure the emotional and social well-being of the assigned constituents are being met, assessed, and documented on a regular basis.
- Attend community events.
Requirements
- Valid Driver License
- Bachelor’s Degree in Social Work from an accredited college or university
- Case management experience
- Experience in leading group facilitations
Qualifications
- No Substitution
- Note: Per Metro Ordinance No. SO94-1078. All employees of the Metropolitan Government shall be residents of the State of Tennessee or become residents of the state within six (6) months of employment as a prerequisite to employment with the Metropolitan Government.
