Social Worker
About the role
Evaluate complex emotional, psychological, and physical patient needs from admission through discharge to determine appropriate social work interventions.
Develop and implement customized care plans based on assessment findings, incorporating patient preferences and aligning with the broader interdisciplinary medical team.
Act as a dedicated advocate for patients' rights, preferences, and cultural needs, ensuring the medical system respects and accommodates them.
Connect patients and families with essential emotional, financial, and social resources to help them effectively cope with their medical diagnoses.
Maintain ongoing dialogue with leadership and the care transition team to continuously evaluate, update, and smoothly execute care plans.

Requirements
- Licensure / Certification / Registration: One or more of the following required: Social Worker specializing in Clinical Practice obtained prior to hire date or job transfer date. Social Worker credentialed from the Maryland Board of Social Work Examiners obtained prior to hire date or job transfer date. Any level accepted. Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. Case Manager credentialed from the American Case Management Association (ACMA) preferred.
- Education: Bachelor's degree in Social Work required.
- Work Experience: 3 years of experience preferred.
Preferences
- LMSW or LCSW-C preferred.
- Experience in an acute care hospital setting, managing high-volume patient caseloads.
- Experience in case management and discharge planning.
- Strong knowledge of post-acute care resources, care coordination, utilization of community services, and interdisciplinary collaboration.

Pay
Salary Range: $66,374.88 - $97,081.63 per year
