Responsibilities

• Work with patients/residents and their family members/significant others to identify strengths, social, emotional, and mental health needs and obtain services to meet those needs.
• Provide services aimed at achieving the highest practical level of physical, mental, and psychosocial well-being and quality of life for patients/residents.
• Ensure services are provided in accordance with the NASW Code of Ethics and compliance with federal, state, and local guidelines, regulations, company policies, procedures, and standards of care.
• Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being, including educating staff on the role of social services.
• Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
• Hold meetings with patients/residents, families, and significant others as requested to provide support and information that improves quality of life.
• Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation, and making referrals to appropriate agencies and suppliers.
• Establish and maintain relationships and referral flow with community-based agencies/services that benefit the patient/resident population.

Qualifications

• Bachelor's degree in social work or human services field required.
• Possess any certifications/licensures required by the State of employment to practice in long-term care (including WV Social Work License).

Pay

Posted Salary Range: USD $24.00 - USD $26.00 per hour.
